What is a Funeral Planning Form?
Your membership packet includes a simple pre-arrangement form for each adult member to fill out, indicating the type of funeral arrangements preferred, such as:
• Direct cremation (without embalming)
• Direct burial (without embalming)
• Cremation following a funeral service
• Burial following a funeral service
• Donation of the body to medical science.

Space is provided on the form for you to record your wishes about a memorial or funeral service, newspaper notices, favorite charity, etc. You may also designate a funeral home.

Is there a list of approved funeral homes?
Yes. The FCAGR furnishes you with a list of carefully selected funeral homes and the prices they charge for each of the above options. You need not select a listed funeral home, however, our price list is useful, should you want to "shop around" among other funeral homes.

What's the procedure at the time of death?
You and the funeral home, if you’ve selected one, keep a copy of the Funeral Planning Form on file. At the time of death, your family notifies the funeral director and authorizes him or her to carry out your final wishes. It's that simple!

What if death occurs outside of the Rochester area?
If there is a FCA affiliate nearby, they can provide a list of their cooperating funeral directors. You'll find a Directory of Affiliates and the handy brochure, "Death Away From Home" on the National FCA’s website (www.funerals.org).

Does the Alliance arrange funerals?
